Construction Alert: Due to ongoing construction starting on June 3, please use the City Hall Parking Garage for your upcoming appointment and allow extra time for parking. There are alternative parking options available nearby. We appreciate your cooperation during the City Hall parking lot upgrades. 

In observance of Independence Day, our office will be closed on Thursday, July 4th.

Passport Processing Times

Due to high demand, appointments are limited. Appointments are released 7 days from today’s date and book quickly, cancellations may make more slots available. Appointments cannot be made by phone. We recommend checking early in the morning as appointments are released daily based on staffing availability at 8:30 a.m.

If you do not see an open appointment slot on our scheduling tool, none are currently available. Another option is to visit the U.S. Department of State Passport Acceptance Facility to search for alternative facilities.

How to Get a Passport

 

Application For A U.S. Passport (DS-11) 

       


Applying in Person for a U.S. Passport
 

  • Complete Numbers 1 Through 22 only in black ink (Social Security Number is required). Do not sign the application.
  • One passport photo is necessary. No eye glasses. No White T-Shirt. The Internet will have listings of available places that take photos. We do not take photos here.
  • Identification is required. A valid driver's license or government-issued ID are acceptable.
    Bring a copy of your ID, front and back, as well as the original.
     

A certified copy of your US birth certificate, original naturalization paper, consular reports of birth abroad, or previous passport is required. Please bring a photocopy of that document as well as the original.

  • We cannot accept hospital birth records with footprints.
  • Your proof of citizenship will be submitted with the application. The original or certified copies of the applicant's evidence of U.S. citizenship will be returned.
  • If you were born in California, you must provide a long form certificate. Short form birth certificates or abstracts are not acceptable.
  • The Department of State will only accept Puerto Rican birth certificates issued on or after July 1, 2010.
  • Your birth certificate must show your parents names.

Persons 16 and 17 Years Old:

  • Must appear with one parent and both will sign the application, and if applicable, provide a driver's license (not a permit) and the parents must provide their identification as well as a copy of the ID for each child. Bring the child's certified birth certificate and previous passport if applicable. The child's Social Security number is required to be entered on the form.

  • You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and must apply using Form DS-11.

Application For A U.S. Passport (DS-11) 

       

 Statement of Consent for Issuing a Passport to a Child (DS-3053)

  • You should complete this form if one parent or guardian cannot go with the child to apply in person for the child's passport. The parent or guardian should submit this special form in addition to their child's application (DS-11). 

       

All children must be present.

  • Children 15 years or younger: Must appear with both parents, legal guardian or persons In Loco Parentis and bring the child's certified birth certificate and previous passport if applicable.
    -If one parent is not available, they may submit a notarized Form 3053 or Form 5525. If one parent is deceased, please bring the certified death certificate.

  • You should complete this form if one parent or guardian cannot go with the child to apply in person for the child's passport. The parent or guardian should submit this special form in addition to their child's application (DS-11). 

       

3.  Statement of Special Family Circumstances (DS-5525)

  • You should complete this form if one parent or guardian cannot locate the other parent or guardian. The parent who is applying should submit this special form in addition to their child's application (DS-11).

       

4. Statement You Did Not Receive Your U.S. Passport (DS-86)

  • You should submit this form only if we completed and mailed your passport and you did not receive it within 10 days of us mailing it. Call the National Passport Information Center and the customer service representatives will walk you through this form.
